Healthcare organization emphasizes support for providers serving some of the country’s most vulnerable

Annapolis, Md., – 2/25/2021 – Curation Health, announced today it has joined the Industry Relations Council (IRC) of the American Academy of Home Care Medicine (AAHCM).  Curation Health will make its suite of services, including an EMR agnostic, prospective risk adjustment and quality platform that simplifies value-based contracting available for AAHCM members. The platform works by driving more efficient HCC capture and care gap closure for patients with chronic conditions.

As a member of the IRC, Curation Health joins a forum for exchanging ideas and insights and for enhancing patient care benefits available to the AAHCM’s nearly 1,000 home care physicians, nurse practitioners, physician assistants, social workers, administrators, and other aligned professionals.

“We are excited for the opportunity to partner with AAHCM on the Industry Relations Council,” said Kevin Coloton, founder and CEO, Curation Health.  “Today, we serve a variety of Home Care Medicine Practices who deliver high-quality care to vulnerable patients suffering from multiple chronic conditions who are also participating in value-based agreements. Managing the complexity of patient care and ensuring appropriate documentation and quality gap closure can be administratively challenging for clinicians. Curation’s advanced clinical decision support platform reduces documentation burdens, empowers clinicians to capture the full complexity of patient care and delivers insights directly into their workflow.”

“We are delighted to welcome Curation Health to the IRC,” said Brent Feorene, executive director, AAHCM. “Their participation in the IRC furthers the Academy’s long-standing commitment to creating partnerships that ensure members have access to best-in-class tools, information, and the support they need.”
